This Notice Describes How Medical Information About You May Be Used and Disclosed and How You Can Get Access to This Information. Please Review This Document Carefully.

About Protected Health Information (PHI)
In this Notice, “we”, “our” or “us” refers to Physiatrix Rehab and our workforce of employees, contractors, and volunteers. “You” and “your” refer to each of our patients who are entitled to a copy of this Notice.


We are required by provincial and federal law to protect the privacy of your health information. Certain types of health information may specifically identify you. Because we must protect this health information, we refer to it as Protected Health Information (“PHI”). This Notice outlines:
– How we use your PHI
– When we may disclose your PHI to others
– Your privacy rights and how to exercise them
– Our privacy obligations
– Who to contact for more information or to file a complaint

1. Treatment
We use and disclose your PHI in the course of your treatment. For instance, after completing your initial assessment or re-evaluation, we may send a report to your referring physician or other healthcare provider. We also maintain records detailing the care and services you receive at Physiatrix Rehab to ensure consistency, accuracy, and compliance with healthcare regulations. These records may be reviewed by our clinical team to ensure optimal care.

2. Payment Involving a Third-Party Payer
We may use and disclose your PHI to bill and receive payment from third-party payers such as extended health insurance providers. This information may include your diagnosis, services rendered, and identifying information required for claims processing. Payers may also request access to your records to confirm the medical necessity of your care.


3. Self-Pay Services
If you choose to pay for your services out-of-pocket without involving a third-party payer, you may request that we do not disclose your PHI related to payment or treatment to any third party.

4. Health Care Operations
We use and disclose your PHI for internal operations such as quality improvement, staff training, accreditation, compliance, business planning, and auditing. For example, we may use PHI to review clinical care outcomes or to train students under supervision.


5. Additional Uses
We may also use or disclose your PHI for purposes related to:
– Appointment reminders and follow-up communications
– Advising you of services, programs, or supplies (you may opt out)
– Coordination of care with third parties such as case managers
– Discharge planning
– Home exercise program follow-up
– Communications via phone, email, or newsletter (you may opt out)
– Non-identifiable research purposes
– Nominal promotional gifts (you may opt out)
– Fundraising (you may opt out)


6. Uses & Disclosures Required or Permitted by Law
We may use or disclose your PHI as required or permitted by applicable law, including:
– If you do not object, to a family member or friend involved in your care
– In emergencies when you are unable to provide consent
– For public health reporting (e.g., communicable diseases)
– For reporting abuse, neglect, or domestic violence
– For regulatory or oversight agencies
– In response to legal proceedings or court orders
– To law enforcement if legally required
– To prevent a serious threat to health or safety
– If required by military command (if applicable)
– For organ donation or transplantation coordination

7. Authorization Requirements

For uses and disclosures not outlined in this Notice, we will obtain your written authorization. You may revoke this authorization in writing at any time, except to the extent we have already acted based on it.

8. Your Privacy Rights
You have the right to:
– Request limitations on how your PHI is used or disclosed (we may not be obligated to comply) – Receive confidential communications at a designated location or number
– Inspect and obtain copies of your PHI in paper or electronic format
– Request an amendment to your PHI if you believe it is incorrect or incomplete
– Receive an accounting of disclosures made over the past six years (exceptions apply)
– Revoke any prior authorizations in writing
– File a complaint if you believe your privacy rights have been violated

9. Our Privacy Responsibilities
– We are required by law to maintain the privacy and security of your PHI
– We will notify you promptly if a breach of your information occurs
– We will not share your PHI except as described in this Notice or as permitted by law
– We will provide you with a copy of this Notice upon request

11. Effective Date
This revised Notice is effective as of 18th April 2025.
